WOMEN'S BOOTCAMP MEMBER / PARTICIPANT UNDERSTANDS THAT IT TAKES 15 DAYS TO PROCESS A CANCELLATION.  IF YOUR BILLING DATE FALLS WITHIN THE 15 DAYS, YOU WILL BE CANCELLED BUT YOU WILL STILL GO THROUGH ONE MORE BILLING CYCLE.  PARTICIPANT UNDERSTANDS THAT THERE WILL BE A $299 DOLLAR EARLY TERMINATION FEE DEBITED IMMEDIATELY FOR CANCELLING THE 12 MONTH PROGRAM.  PARTICIPANT UNDERSTANDS PHOTOS OR VIDEO MAY BE TAKEN DURING THE COURSE OF THEIR INVOLVEMENT IN WOMEN'S BOOT CAMP, WHICH MAY BE USED FOR PROMOTIONAL PURPOSES.  GUARANTEED RESULTS REQUIRE PARTICIPANT WOMEN'S BOOT CAMP MEMBER TO FOLLOW OUR PROGRAM WHICH CONSISTS OF EXERCISE AND NUTRITION.  THE PROGRAM REQUIREMENTS ARE AS FOLLOWS:  PARTICIPANT WOMEN'S BOOT CAMP MEMBER 1) MUST RECEIVE BASELINE CIRCUMFERENCE MEASUREMENTS AND COMPOSITION MEASUREMENTS BY A QUALIFIED WOMEN'S BOOTCAMP REPRESENTATIVE AND COMPLETED WITH IN THE FIRST WEEK OF THIS DATED AGREEMENT, AND MUST REMEASURE EVERY FOUR WEEKS AND DOCUMENTED IN CLIENT FILE.  2) GOALS MUST BE OUTLINED, DEFINED AND DOCUMENTED IN CLIENT FILE. 3) MUST FOLLOW THE RECOMMENDED NUTRITION PROGRAM AND MUST KEEP ALL NUTRITION APPOINTMENTS UP UNTIL THE LAST DAY OF MY PROGRAM. 4) My success journal food diary must be kept current from the day of this dated agreement to the day of my weigh-in nutrition apointment and continuously up until the last day of my completed program.  5) have attended all bootcamp classes consistent minimum 3 times a week every week during the full term of my program.  6)  I must do my cardio vascular program mINIMUM 5 times a week consistenly through the term of my PROGRAM.  7)  I the buyer may be subject to a polygraph TEST if i have satisfied all aforementioned requirements (sections 1-6) and have not reached the guaranteed goal by the completion of the program by the women's boot camp.
